SilverDragon tracks rares. It'll remember where you last saw a rare mob, and will pop up a notification message when you get near one.
It comes with a list of rares. Type "/silverdragon" to bring up the options, and use the "Import Data" button to bring 'em in. (It's not installed by default, in case people would prefer to keep the suspense alive. )
If you have nameplates enabled it'll scan through them to find rares nearby. Otherwise it requires you to mouseover/target a rare, or for your party member/pets to do likewise.
When a rare appears, SilverDragon will pop up a frame that you can click on to target it.
SilverDragon can also create a macro to target any rares that are known in the current zone. It's just that good.
If you import the default dataset: the very first time you come within visual range of a rare, SilverDragon will tell you. It only works the first time because it's checking the game's mob cache, and once you've seen it the game will remember. Remember, this won't happen if you don't do an import.
You can clear the mob cache by quitting WoW and deleting the Cache sub-directory in your WoW directory.
A LDB plugin is included to list rares in your current zone. I like ButtonBin as a viewer, but there'll be a minimap icon if you don't have any viewers installed. (You could use Broker2FuBar to put it on FuBar.)
If you have Cartographer or HandyNotes installed it will add map notes for rares, which can be shared with others.
Note that if you don't have a Broker viewer, and you don't have a supported map notes addon, all this addon will do is print a message into your scrolling text when you mouse over a rare!

There is one rare mob in the Dalaran Underbelly that cannot be killed or tamed, and you can only get to it through a glitch with the graphics and an underbelly elixir.
I would also like to have Vern removed, he is unkillable, I almost said unreacable but you can get to him if you are creative, just can't kill him so no sence having him in there is there?

For the record, the same thing has happened to me. I use a BugGrabber program, and the error that popped up was the following:
I presently have a number of cached rares, primarily old world rares (I had been leveling a new character). The rare it choked on for me was Krellak, in Silithus. It has happened in the past with other rares, and the glitch was in the same location, even though it was a different rare each time.

As far as I know, there are not any appearance alrenating addons being run, Now I will say I seem to get it after seeing and killing a few rares, if that

First, I love this mod but was wondering if it would be possible/worthwhile to disable cache scanning while in flight (not on your own flying mount). I often get cache detections while flying over a zone and since I can't hop off and find the mob it's a little bit distracting. Not a big problem but just a suggestion. Thanks for the addon.

As was said, it's possibly a false positive from a tamable mob.
Cache alerts won't appear until you're in the appropriate zone, so if you managed to trigger the cache filling up before entering that zone, the mob might be nowhere near you. Wandering by a hunter with the mob tamed is one way for that to happen... but not the only way.
The cache scanning isn't perfect -- something appearing in the cache isn't a guarantee that it's anywhere near you. It's just fairly likely. That's why I had the alerts include the source of the alert. (Also why there's an option to disable alerts for tamable mobs.)

Don't quote me on this, because I don't know which mobs are dinging, but for me this usually means that it's a 'false positive' on a tamed rare mob. I get this happening a lot with Loque, for instance.

Nearly forgot, got a false positive yesterday:
Actually found Syreian the Bonecarver, who patrols in eastern Grizzly Hills, and when I was farming Dragonfins later, I flew into Grizzly Hills from the West, from Dragonblight, just south of the large lake in the north, and suddenly got her frame again the moment the zone changed. Thought it could be a caching bug because the addon maybe noticed Scarlet Highlord Daion, though I turned around, but couldn't find her.
Since the updating of the images, I've been having problems with some of the popup model views of the critters. Instead of being a nice model, they are a huge digitized lump of a closeup of their left nostril or something similar. The 'Faulty War Golem' was the last one of these I saw it happen to, though there have been others. Some of the models don't seem to come through clearly on the portrait views, either, instead being a semi-transparent colorless view.
In addition, the models used to rotate, from what I remember. Now they simply stare face-on. That might be an intended change, mind you; I'm not positive.
