 Missing Maelstrom

I'm glad I found this addon. I sometimes get consumed in what i'm doing and miss when one of my weap enchants wear off or my shield is gone. However, if I understand your discription correctly, I should see another icon when Maelstrom has stacked to 5. But, I do not. Any help would be greatly appreciated.
