For Ultra / Madness, you are just supposed to get the extra button as a seperate frame on the screen and it doesn't touch your cast bars as you still need them.
For DMF etc then yes replace the bar...

Thats great! edited the _player.lua and _target.lua and with the latest 5.07.18 everything is working nicely for me.

Just on the 5.07.14 when I downgrade, that seems to be the only issue, but as I said I'll live iwht that as I can see the hud debuffs...

Sounds sensible, but the way I've found it to work is this.
Upgraded via each release to the latest and have this issue.
Deleted the nui folder and restored the 5.07.14 release.
Now the buff bars on...

Hud Buff problems

With nUI 5.07.16 and 4.0.6, the hud buffs no longer display the mouse over information. Self and target buffs seems to work correctly.
