I had this issue when I turned on the ToT tooltip...

I had this issue when I turned on the ToT tooltip with tinytip. (I think that was the name of the option, been awhile). After I turned it off and did a quick reloadUI then the issue was gone.

Diffrent AddOns can allow you to move that box. TinyTip and CowTip are 2 that come to mind. I use Cowtip because it seems easier to me to place the tool tip right where you want it.
