i have the same problem on hunter, warlock and mage. if you summon a pet in combat is when the problem is seen reliably.
the conflict i have found with with a large list of mods i have including
ct raidassist
mapnotes
tastethenaimbow
cooldowncount
partyquests
lots of other cosmos ui mods
the list goes on but i cant remember all of them
