That right there is the largest issue. You really need to update to the latest version which is 5.06.17

For your graphic issue, I'm fairly certain it was a video driver bug of some sort that made most UI elements have a white outline and the normal HUD bars disappear and only show shaded rectangles. This only happens when you go into and out of Windowed mode. The only fix is to reload your UI. Easiest way to do that is using the command "/nui rl" minus the quotes of course.
