Unknown PVP Cooldown addon

Hi, I'm trying to identify a specific PVP addon that shows icons when a player pops a cooldown, has a specific spell on him, or gets CC'd. I think there can be as many as 4 icons for each player and the icons follow the player when they move. Also, the icons countdown the duration of the spells.

Afflicted can be configured to use icons.

The icons on the image are on the nameplate. As far as I know, afflicted cannot be configured to show on the enemy nameplates.
