Anyone know what mod this is?

It's probably a stupid question, but I'm looking for a nice, basic mod for my GUI and this screenshot is basically EXACTLY what I'm looking for.

Basically everything that is customized from the original GUI.

Sorry for not specifying that earlier.

For example, that bar on the top of the screen, the XP bar next to the map, how he has icons above the bar on the bottom & the right. I'm sorta new to WoW, so if these can be done without mods, please let me know

The extra buttons are part of the default interface and can be added in the interface options section of the main options menu.

The bar at the top of the screen is Titan panel (i think). You can also use FuBar. Both are referred to as panel mods that you plug other mods in to.

I cant recall the name of the xp bar unfortunately.

Fubar can be gotten from wowace.com Titan should still be available at curse.com.

Not sure on the rest but there is a good start.

Personally I prefer fubar to titan panel and cartographer to both atlas and atlas loot, but I have used everything mentioned here, they are all good, but some just work better for me.
