Can anyone identify this interface?

Hi everyone, I just watched some PVP videos of Jedion and I've been trying to figure out what interface mod he uses, and if it still exists. Here's a link to one of his videos, the mod I'm interested in is how it displays his health and the enemy's health next to eachother.

Looks like Discord Unitframes, Titan bar.
Not sure what action bars the guys using.

Can't identify anything else sorry.

This is just a custom UI made by that guy in Discord, He moved his personal and target frame down and eliminated all the backgrounds. Its pretty easy to do, but discord hasn't released a new version for 2.0 yet. Other than that its just Titan with his action bars shrunk down and his hotkeys remapped to the [f1]-[f12] keys and the usual 1-=.

Thanks for the replies, forgot to mention the main thing that I wanted to know about was the health bar mod that put them at the bottom next to your targets health bar.

I'm not sure, but if it is discord. I'd like to know how to set that up. I like the two screens at the bottom with the percentage of health. That's something I would definetly be interested in.
