Some buttons disappearing occassionally, and attack button becomes a ? occassionally

For some reason, some of my buttons keep disappearing, they still activate if I hit the keybinding, but they dont show on my button bar, and I cant put new spells there either, changing the "self-cast" options seems to bring them back, as does simply shifting out of that bar into a different one and back. No bugsack errors tho. Also, sometimes when i log in, my attack button just becomes a big red ?, only way to fix it, is to remove the action button, and replace attack there.
