Had not noticed any of this on my Warlock but found it on my Druid with the Eclipse mechanic.
Setup:
Action bar has both Moonfire and Sunfire as independent buttons with different key binds; no special stance/phase shift in bar.
What happens:
As the sun/moon shifts the horizon and spell glow activates on the bar the Moonfire has icon shifted to Sunfire, does not cast Moonfire and will not revert back until next Eclipse.
