Bug with talent spells.
There is no icon support for talent spells. When you first open item, spell or achievement link and after that open a talent link it will display the icon of the last opened link.
Click on the link bug.
If you click new link, when some link is opened already, it closes existing one and after that you have to click again to open the new link. It should close existing one and open a new one on the same place.
Achievment icon frame bug.
The achievement icon frame doesn't disappear after opening a new link. It displays around the icon of the last opened spell or item link.

The talent spell issue is a known problem, but sadly there isn't anything I can do about it. To get the spell icon, I need to know the spell ID. However, talent links only give me a "talentid" which I can't correlate to a spell id. Requests have been made to improve the tooltip API for spells, and hopefully I'll be able to support talent spells soon (like maybe 3.1).
The other two I cannot reproduce. I strongly suspect that they're the result of other addons screwing with you.
