WoW 2.0 finally added the ability to detect swimming and you'll find that the current version of Outfitter (1.4.6) happily supports a swimming outfit now :)

Please provide exact steps for the error you're seeing -- I'm not able to generate any errors while trying to swap weapons.
I was able to reproduce the problem with sharpening stones however, I'll take a loot at that and figure out the problem.
