 Custom event only working some times ???

Having an issue with Concussive Barrage (a chance to stun target on regular shots). I finally figured out how to detect it but it is only working *some* times. Maybe 30% of the time SCT will show my custom alert (and play a sound), while the other 70% of the time it procs it does nothing. I am using:
