It works fine, then you die, then when you look at the map while dead it shows the wrong map? Does it work correctly when you are alive again?
For the Feralas issue, all you have to do is fly from the flight point north...

I got it to reoccur. But what you have to do, is die in stonecore, then go back in. Otherwise the map works fine.
I also found another spot metamap has problems with the map with -- Go to "Tower of Estulan, Feralas" Flight poi...

If possible, can you go back to stonecore and try again (after you have quit WoW and come back in). If it still does it, can you disable all addons but metamap and try again?
Can you also make sure you are using the latest MetaMap?
Went in today, it worked just fine. Maybe a blizzard glitch then.

When going into stonecore dungeon in deepholme, I cannot view the dungeon map, because Metamap keeps zooming out to the "azeroth" view (i.e., showing both outlands and the vanilla world/northrend world view). The dungeon map shows up just briefly. This is the only cata instance I've seen this issue with so far.

Unable to see completed quests

I made sure the checkbox to hide completed quests was unchecked, and then did a server query, but it fails to show the quests I have completed in the zones I look at (And I'm using outland & northrend quests, so they aren't cataclysm affected). Did I miss something?

I notice that when I go into an old WoW instance (like Stockades, deadmines, etc), the map that gets pulled up just shows the current zone rather than a map of the instance, which I've always found useful. I use Atlas as well to get the map, but it'd be nice to see the "M" key work for it too.
