Unit Frames show this, but if you want something more noticeable, MSBT is probably your best choice.

1. /msbt
2. Triggers
3. Add New Trigger
4. Name it something like "Target 35%"
5. Main Events: Add Event
6. Main Event = Health Change
7. Click spanner next to Unit ID, change to Unit ID = Target
8. Click spanner next to Threshold, change to 35
