I'm sure a lot of you know about ForteXorcist, in my eyes the single best Warlock addon out there.
One of the warlock-only functions it has is announcing when a CC is about to end, which I think is completely awesome. Unfortunately, since it is warlock-only, it won't work for my mage's sheep (or penguin, as it were). Does anyone know of an addon (that's been updated for Cataclysm of course) that could announce in /p or /ra (whichever is appropriate) when Polymorph is five senconds from expiring, as well as when someone breaks it early? Extra plusses if you can display the name of the person who broke it.  And if there isn't one, can one be coded? Puh-lease?

Forte has support for all classes, including announcing when CC breaks/is fading.
