What UI Mod is this?

There is a screenshot of a really cool UI Mod I'd like to use. Does anyone know where I could get it? I'm currently using Cosmos.

I think that the mod on the left could be Kaitlin's MiniGroup, but I could be mistaken.

I can't seem to find that, but I've been searching for a mod that organizes party members and targets like that mod on the left seems to do. I also already found out that the mod on top is Titan Bar, but I'm really interested in what makes those party, self, and target boxes.

Those are Nurfed UnitFrames, part of the Nurfed-UI. You can download the entire package here. Extract and only install the UnitFrames if that is what you want.
