I suspect this may just be due to no lock feature presently in place -- though I do love the lock during combat for buttons -- but occasionally if I have a spell selected to use, and then I mount up on a vehicle, die, or am otherwise put into a situation where the selected spell could not be used (meaning that there's the highlighted cast cursor waiting for a target), the spell will not only cancel, but the associated button will disappear from the toolbars.
It's a bit sporadic, but that's the best I can nail it down presently. If I can determine more, I'll add to this.
