I am betting it is either
EventAlert or
Ghost: Pulse. I run Event Alert myself and find it very useful and that looks similar to what event alert looks like but if I recall I dinked around with ghost pulse and his has options for cooldown spell icon pop up action as well. Hope I have helped you out!
